Nurse Support

Your nurse is part of the team that is here to care for you. This program provides a nurse case manager for your ongoing support. Nurse case managers can help with:

  • Assess your health status
  • Answer questions about your medical conditions
  • Provide education and resources on your medical problems
  • Locate health care services

Please call your nurse with any questions or concerns. You can reach Customer Service through the toll free line 888-327-0671. Customer Service can answer your questions or they can direct you to your nurse.
